INSTRUCTIONS FOR NOMINATORS
Please complete the application below and submit a narrative telling us about your nominee and why he/she should receive an award. Be as specific as possible providing actual examples of student’s academic achievements, extracurricular activities, personnel qualities and character, creativity or talent, demonstrated leadership abilities and personal barriers overcome by the student. Please type your essay and attach to the application. You can also handwrite the essay on the back of this sheet. Attach documentation including transcripts (i.e. SAT, PSAT and SAT2) and awards to this application. Write candidate’s name on each page. ALL APPLICATIONS WILL BE SHREDDED AT END OF SELECTION PROCESS. HOWEVER, ALL OF THE STUDENTS’ DEMOGRAPHICS WILL BE SAVED BY THE LACC. THIS WILL ENABLE US TO UPDATE THE STUDENTS’ INFORMATION ON A YEARLY BASIS AND FOLLOW-UP WITH SCHOLARSHIP OPPORTUNITIES.
